Activity Waiver, Release, and Assumption of Risk

8/22/25 – 8/23/25 – Cornerstone BBQ Bash Activities

In consideration for being permitted by the Beaumont-Cherry Valley Recreation & Park District (“District”) to participate in the activity referenced above, I agree as follows:

1.      Waiver and Release
I waive, release, and discharge any and all claims for damages for personal injury, death, or property damage which I may have, or which may later accrue to me, as a result of my participation. This release is intended to discharge the District, its officers, employees, volunteers, and agents from all liability, including liability arising from active or passive negligence.

2.     Assumption of Risk
I understand that participation involves risks, including but not limited to personal injury, death, communicable diseases, illnesses, viruses, or property damage. I voluntarily assume all such risks.

3.     Indemnification
I agree to indemnify and hold harmless the District, its officers, employees, volunteers, and agents from any loss, liability, damage, cost, or expense arising out of or connected with my participation.

4.     Medical Authorization
In the event of accident or emergency, I authorize District staff or volunteers to obtain medical treatment for me/my child and agree to be fully responsible for any resulting expenses.

5.     Photographic Release
I understand photographs or video may be taken during this activity. I grant the District permission to use such images for lawful promotional purposes without compensation.

6.     Parental/Guardian Consent (if participant is under 18)
I, the undersigned parent/guardian, consent to my child’s participation and certify that my child is physically able to participate. I execute this waiver and release on their behalf and agree to indemnify and hold harmless the District as stated above.

Acknowledgment
I HAVE CAREFULLY READ THIS AGREEMENT, FULLY UNDERSTAND ITS CONTENTS, AND VOLUNTARILY SIGN IT AS A RELEASE OF LIABILITY.
